This secure form collects the information Clearview needs to assess your subcontractor registration.

• Every visible field must be completed.
• Select at least one province and at least one service.
• Complete every contact and identify one Primary Contact.
• The Submit Registration button unlocks only when the form is complete.
• If an ordinary text field does not apply, enter N/A rather than leaving it blank.
